Continuing Care & Health Service

 

Service Introduction

Our clinic was established in 1958. Aside from providing medical services, we also strive to promote health awareness in the community. Throughout the years of development, in addition to general out-patient services, we are now equipped with various specialist out-patient services, health services, and health education services, to match the needs of medical services development and community.

We also provide quality residential care services to seniors requiring continuous care. In 1968 we founded Wah Hong Home for the Elderly in Wah Fu Estate which was the first elderly home established in a public housing estate. Then Shun Lee Home for the Elderly and Cheung Fat Home for the Elderly were founded in 1981 and 1991 respectively. Currently the 3 elderly homes provide 178 places for the seniors. Moreover, our elderly homes also provide respite placements, allowing caregivers to relieve pressure stemming from taking care of their seniors for a long period.

To facilitate "ageing in place", Chin Wah Day Care Centre for the Elderly and So Uk Day Care Centre for the Elderly were established in 2002. So Uk Day Care Centre for the Elderly was then moved to Un Chau Estate and transformed into Sham Shui Po Day Care Centre for the Elderly. Both centres also provided respite day care services to support the seniors and their caregivers.

In 2019 we implemented the Pilot Scheme on Multi-disciplinary Outreaching Support Teams for the Elderly (Kowloon West Cluster) to provide rehabilitation and social linkage services to users residing in Shamshuipo's private residential care homes for the elderly (RCHEs). The team also provide speech therapy services to the contract RCHEs, self-financing RCHEs and self-financing Nursing Homes. The scheme was regularised in 2023 and renamed as Multi-disciplinary Outreaching Support Team for the Elderly (Kowloon West). 

 

 

Service Objectives

With the core value of people centeredness and caring for the whole person, we deeply believe that our seniors deserve a dignified, active, and delightful life. We also believe that the physical and psychological wellbeing of community members and caregivers has to be cared for. Thus, through our transdisciplinary collaboration, combined with having a united team, we endeavour to meet the following objectives:

  • To promote health awareness in the community and among seniors, to improve and enhance their physiological and psychological wellbeing;
  • To enhance/ maintain seniors' bodily functions and independence, allowing seniors to enjoy a dignified life with self-worthiness;
  • To provide seniors with continuous and appropriate care and social service, allowing them to enjoy their golden years;
  • To provide assistance to caregivers, and facilitate the connection between seniors and their family members, allowing both parties to enjoy familial love.
