Day Care Centre for the Elderly
Service Objectives
By adopting the principles of "continuum care" and "community care", and with "client-centre" as the service goal, we aim at:
- Enabling frail seniors to maintain an optimal level of functioning by day care services to meet the nursing, rehabilitation and social needs thereby enhancing their quality of life.
- Providing support to caregivers of the seniors to enhance their confidence, ability in performing care duties and relieve stress.
Introduction
Our "Chin Wah Day Care Centre for the Elderly" (CWDE) and "Shamshuipo Day Care Centre for the Elderly" (SSPDE) under Long-term Care Service are situated in Kwun Tong and Shamshuipo respectively. We aimed at providing service to frail elders aged 60 or above and their caregivers. The 2 centres provide support to over 200 service users annually. Major types of services include:
Personal care
Provide daily personal care, such as feeding and bathing, etc.
Nursing Care
Provide basic and special nursing care, including health consultations, regular health checks and health talks, etc. by professional nurses.
Restorative and Maintenance Rehabilitation Exercises
Service users will be guided by physiotherapists and occupational therapists to do rehabilitation exercises.
Counselling Service
Provide emotional support and referral services to service users as well as their carers by professional social workers.
Social and Developmental Activities/Groups
Different kinds of activities and groups will be organised to develop potential of the service users.
Transportation Service
Rehabus service is provided to enable service users to travel from the pick-up spots to the centre and vice versa.
Meal Services
Balanced meals are provided for service users daily. Special diets will be given according to service users' health conditions.
Carer Support Service
Resources corners have been established for carers. A wide variety of supportive services will also be provided to enhance carers’ confidence and abilities.
Extended Hours Service
Service is provided for needy service users from 7:30 a.m. to 8:00 a.m. and from 6:00 p.m. to 6:30 p.m. from Mondays to Saturdays.
