Wan Hon Integrated Home Care Service Team

 

Introduction

"Wan Hon Integrated Home Care Service Team" (WHIHCST) was established in 1997. During the preliminary stage of service provision, while the site in Wan Hon Estate was under construction, a provisional office was set up in Sau Mau Ping Community Centre to provide home care services for the needy. Upon the completion of Wan Hon Estate, the "Wan Hon Integrated Home Care Service Team" was moved into the new office premises since then.

 

 

Service Purpose

Integrated Home Care Service Team is a home-based community care and support services providing a wide range of care and services to the elders, the disabled, individuals and families with social needs, so as to actualise the rationale of "ageing in place" and "continuum of care".

 

 

Service Objective

To help service users to achieve and maintain an optimal level of functioning and independence, and to prevent premature and inappropriate admission to hospitals and residential care.

 

 

Service Target

Home Support Services Cases

  1. Elderly persons aged 60 or above living in the community and not receiving institutional service; and
    Assessed to be at the state of mild impairment or requiring higher level of care under a designated assessment tool; or assessed and recommended for Home Support Services under the Standardised Care Need Assessment Mechanism for Elderly Services.

  2. People with disabilities and individuals and families with social needs: Have to be referred by registered social workers from social welfare service units.

 

Frail Cases

  • Aged 60 or above living in the community and not receiving institutional service; and
  • Assessed and recommended for Community Care Services or Residential Care Services under the Standardised Care Need Assessment Mechanism for Elderly Services. 
  • Carers of the service users of IHCS (Frail Cases)

The scope and frequency of services will be designed by our service team according to the needs of individual service users.

Besides the services mentioned above, we also organise volunteer visits, social activities and make service referrals so as to enhance service users' social networks and enable them to have positive outlook of life in the community.

 

 

Fee

Service users are required to make co-payment with amounts determined on the basis of their household income, and may be revised annually.

 

 

Application for Service

Home Support Services cases

Applicants who aged 60 or above can approach respective Integrated Family Service Centres/ Integrated Services Centres, Medical Social Service Units, District Elderly Community Centres, Neighbourhood Elderly Centres or Social Centres for the Elderly for referrals or Integrated Home Care Services Teams (IHCSTs) direct for application.


People with disabilities and individuals and families with social needs have to be referred by registered social workers from social welfare service units for application.


Frail cases

Applicants can approach respective Integrated Family Service Centres/ Integrated Services Centres, Medical Social Service Units, District Elderly Community Centres, Neighbourhood Elderly Centres or Social Centres for the Elderly for referrals and arrangement of Standardised Care Need Assessment for Elderly Services. Termination of Service

Self-withdrawal

Service users can inform our social workers for termination of service. 

Discharge arrangement

Home Support Services cases

  • Services will be terminated according to the result of service assessment.  

 

Frail cases

  • Services will be terminated for the following conditions:
  • Self-withdrawal, suspension of service over 30 days (such as long hospitalisation or departure etc.), improved self-care ability, other support network available, institutionalisation, deceased.
